How did you make out with your book cover? I'm so sorry you are having great difficulties I wish I could give your more help. But do you really need your book to be hard cover with dust jacket?
I ask that is because I personally don't like books with dust jackets. I often get deals from books in Double Day Book Club, and all they ever have are books with dust jackets, and I use to just hate the jackets. Whenever I read the books, I had to take off the dust jacket to put them somewhere and just hold the book in my hands to read it. I don't know the size of your book, but if you are using lulu.com to publish your books, I happen to know that their books in PERFECT BINDINGS are most attractive in compared to other publishing companies of the same sized books.
I really do not know your purposes for wanting to do this in hard covers, for it would be more money for shipping and handling. Perhaps you should reconsider the alternatives for the book covers. I'm specialized in covers of books in size of 6 x 9" PERFECT BINDING, I can even send you the format template of the cover in perfect binding, with pixels and size all set up for you all you'll need to do is to just upload your pictures there: once for the front, and once for the back.
And lulu will put them together for you, and you'll just need to click on the link where it says "Spine" to put in the text of your spine.
Please let me know whether you wish to reconsider in the making of your book. But I guess if your preference is on hard covers, then of course you would need a dust jacket, but I'm not familiar with creating dust jacket, I don't want to have to mess it up for you for each time you mess up something you'll have to go back to make corrections and then upload your new corrected version again, and you'll be prompted yet to buy ANOTHER copy for your proof again, and it will become costly for you if we need to correct the dust jacket several times.
I, myself, have bought about 4 times of Aunt Sandie Goes To School for proofing, so I speak with experience to warn you of what would happen if your cover isn't right and the money you'll have to pay out before you even have the chance to send it for printing.
I hope you get my point of view. I didn't mean to offend you or anything. I was just trying to help you so you won't have to spend the money for proofs like the way I did.
I hope someone who has the experience in doing dust jackets in books will come along to give you a better advice than I did.

I'm so sorry you're having so much difficulties in finding someone to do the illustrations for your book cover. I think the reason is that this is because your book cover being a dust jacket which a lot of us are not familiar with, and as you can tell from Linda Perry's posts how many times she has had to order for her proof copy before she finally got her book right, and I'm just thinking that if I had to do this for you and I don't want you to have to order many times to get the book cover right.
But if you really don't mind about that just let me have a lot on your template and the picture you have chosen.
You cannot make covers unless you have Adobe Photoshop, I'm in the middle of doing two books for myself, but I can take a look at the template that you have got see if I can find out how to do it, but like I say I cannot guarantee anything it's the first time I do dust jacket, measurement and everything that I'm not familiar with.
I do not recommend people doing hard cover it takes a lot of money for shipping and handling, and I don't even know how the cover quality is in lulu for dust jacket.
I can tell you PERFECT BINDING are the best in lulu, they make it beautifully-laminiated, but you only want hard cover with dust jacket which makes this a hard task on me, and I don't want to have you order about 5 times to make every looks right, but if you can risk your money then I have no problem in looking into this for you.
Perhaps there are some other more professional authors who has done the hard covers with dust jackets already, so perhaps they too can help as well.
But please consider the shipping and the handling cost for each book being shipped to you before you get everything in the right place.
